Approaches to the treatment of patients suffering from chronic obliterating diseases of lower limb arteries under ambulant conditions are discussed. The authors hold that the given trend is most important for the attainment of long-term results including those after surgical interventions on vessels. The main cause of insufficiently effective medical aid rendered to such patients consists, in the authors' opinion, in the lack of trained internists specialized in angiology. The results of ambulant treatment of patients with chronic obliterating arteriopathies are provided.

The paper provides the results of multimodality conservative therapy in 527 patients with chronic obliterating diseases of extremity arteries, which was performed in the outpatient setting. Factors influencing its efficiency were analyzed. The paper emphasizes the value of this direction of treatment whose underestimation sharply aggravates the prognosis of these diseases, including the results of their surgical management. Having obtained positive results in 84.5% of patients, the authors note the efficiency of treatment to be decreased with severe arterial insufficiency of lower extremities (5% of negative results in Stage IIa, 23% in Stage IIb, and 40% in Stage III), in patients with prior surgical interventions, and in elderly patients with atherosclerosis obliterans and younger patients with non-specific aortoarteritis.

Case records of 2135 outpatients referred to a specialized department with a provisional diagnosis of limb arterial affection have been analyzed. It appeared that in 35% of the cases the diagnosis was erroneous, the complaints being due to neurological and locomotor disturbances. Nosological entities of confirmed vascular diseases were the following: obliterating atherosclerosis (81.8%), obliterating peripheral aortic arteritis (9%), obliterating atherosclerosis associated with diabetic angiopathy (6%), obliterating thromboangiitis (1.6%), Raynaud's disease (1.6%). The disease-specific age pattern showed that obliterating atherosclerosis and age-specific cholesterol levels have the same trends, that there may be transformation of obliterating thromboangiitis into nonspecific aortic arteritis which eventuates in advanced age into obliterating atherosclerosis.

The paper outlines the analysis of immediate and late outcomes of transcutaneous transluminal angioplasty (TTA) in 147 patients with atherosclerosis obliterans of the lower extremities. The study has revealed that age and dyslipidemia as TTA risk factors fail to affect the immediate outcomes in the early postoperative period. There is a positive correlation of the reocclusion time after TTA and the age and lipid metabolic disturbances in patients with atherosclerosis obliterans of the lower extremities in long-term periods. The paper provides strong evidence for that it is essential to correct dyslipidemias in order to make TTA results better in these patients.
